Crestview Kids Individual Discipleship Plan

Every child is different, and we want to do our best to meet each child where they are while coming alongside parents as they disciple their children. Because of this, we have created an IDP (Individual Discipleship Plan) that we can develop together with all parents.

Our goal is not to put every child into the same box. Our goal is to understand each child as an individual. We want parents to know that they don't have to navigate these things alone. We want to come alongside you, support you, and work together to help your child grow in the Lord.
Our Purpose
Our purpose is to break down barriers that keep children and their families from fully participating in the life and community of the church while more effectively serving the entire Body of Christ.
This Individual Discipleship Plan (IDP) helps our ministry team partner with your family as we seek to understand, support, and disciple your child. Because every child is uniquely created by God, we want to learn what helps your child feel safe, loved, supported, and ready to learn about Jesus. We also want to understand how we can best support you as parents, recognizing that you are your child’s primary spiritual disciplers. Our desire is to come alongside your family and work together to help your child grow in their relationship with Jesus.

CRESTVIEW MINISTRY ACTIVITY, TRANSPORTATION & MEDIA WAIVER

I give permission for my child/children named above to participate in children’s ministry activities, programs, events, outings, and other church-sponsored activities conducted by Crestview Baptist Church during the period covered by this form.


I understand that some activities may involve ordinary risks associated with participation, including but not limited to transportation, recreational activities, games, sports, outdoor activities, trips, and other ministry events. I understand that reasonable safety precautions will be taken by Crestview Baptist Church and its leaders and volunteers.


To the extent permitted by law, I agree to release and hold harmless Crestview Baptist Church, its staff, ministry leaders, volunteers, and representatives from claims arising from my child’s participation in church activities, except where prohibited by law or resulting from gross negligence or intentional misconduct.


TRANSPORTATION CONSENT


I give permission for my child to be transported in church-owned vehicles or vehicles operated by approved church staff or volunteers when transportation is necessary or appropriate for a Crestview Baptist Church activity or event.  I understand that transportation may include travel to and from church activities, retreats, camps, service projects, outings, and other ministry-related events.


PHOTO & VIDEO CONSENT


I give permission for photographs, video recordings, and other media of my child taken during Crestview Baptist Church activities to be used by the church for ministry-related purposes.  I understand that my child’s name will not ordinarily be published with photographs or videos unless separately authorized by the church or included as part of a ministry communication.


PARENT/GUARDIAN ACKNOWLEDGMENT


By signing below, I confirm that I am the parent or legal guardian of the child named above and that I have read and understand this form. I voluntarily give the permissions described above and understand that this consent remains in effect for Crestview Baptist Church activities from October 2026 through December 31, 2027, unless I provide written notice to the church withdrawing my consent.xt...
